There are currently two formats for disposing of chemical waste:

• Hazardous Materials Pickup Request Form in non-interactive Adobe PDF (requires Adobe Acrobat Reader)
• Hazardous Materials Pickup Request Form in Excel spreadsheet (requires MS Office or Excel)
• Each chemical container should be labeled with a index number that corresponds to your sheet
• Fax to number given on form
• Materials will be picked up by OU OEHS
