Note that the first page addresses state-level needs that target state staff and volunteers, state partners, and Training/TA subcontracts. The second page addresses the needs of local preventionists. Feel free to contact Carrie Sparling (405-325-0778) with any questions about the form. Thank you for your cooperation!
