Concurrent Enrollment RequestThis form is required for undergraduate students pursuing more than one degree.
University Petition to Drop a Course After the DeadlineThis form is required to drop a class from your schedule after the official enrollment deadline. Students must also attach a "Drop Form" signed by the instructor of the class. The Drop Form is available in each School's main office or in the Fred Jones Art Center, room 122.
WFCFA Internship Credit FormThis form is required for students completing an internship for undergraduate credit in College of Fine Arts.
WFCFA Course Time Conflict FormThis form is for undergraduate students needing to enroll in classes with a time conflict.
University Enrollment Overload PetitionThis form is for undergraduate students wanting to enroll in more hours than the allowed term limits.
