Permission Request for Center for Independent and Distance Learning CourseThis form must be completed and submitted to the Fine Arts Dean's Office in FJC 122 for permission to enroll in a course through the Center for Independent and Distance Learning.

University Enrollment Overload PetitionThis form is for undergraduate students wanting to enroll in more hours than the allowed term limits.
College of Fine Arts Petition to Add a Course after the DeadlineThis form is required to add a class to your schedule after the official enrollment deadline. Students must also attach an "Add Form" signed by the instructor of the class. The Add Form is available in each School's main office or in the Fred Jones Art Center, room 122.
University Petition to Drop a Course After the DeadlineThis form is required to drop a class from your schedule after the official enrollment deadline. Students must also attach a "Drop Form" signed by the instructor of the class. The Drop Form is available in each School's main office or in the Fred Jones Art Center, room 122.

Art History MinorStudents must declare this minor in the Fine Arts Dean's office, in addition to completing this form with an Art History faculty advisor.
