INSTRUCTIONAL LOAD DATA REPORT

Each semester a Missing and Invalid Instructors report will be sent to each department. This report shows only the classes that have enrollment but some necessary information is missing. Either the instructors social security number, name, office room number and/or building is not listed or the Personnel Action Form needs to be updated with the Provost's office. This information is required in order to complete the information on the Instructional Load Data Report (ILDR) which is sent to the State Regents as a description of the instructional activities at the University.

Please read the following instructions carefully. If you have any questions concerning this material, please call the Office of Classroom Management, 325-1087.

  1. ROOM NUMBER AND BUILDING: Verify the accuracy of the room and building listed for the course. If the room and/or building are missing or incorrect, please change to the instructor's, not the departmental, office. At this time departments are not allowed to enter the building and room numbers on the computer, so please enter the correct information on the report.

  2. INSTRUCTOR SOCIAL SECURITY AND NAME: Verify the accuracy of the instructor(s) listed for the course. Check for both social security number and name. If either is missing or incorrect, please first use your SKUP skills to make the corrections needed. If only the name does not appear after the SS#, please write in the instructor's name and title on the report. Also, if the name does not appear after the SS#, or under the heading ERR you notice “TRM”, this should alert you to check on your instructor file in the Provost office. This applies to sections taught by graduate assistants as well as faculty, and should include multiple instructors (team taught) when applicable. Remember, list the instructor who actually teaches the class; do not use program directors unless they are also teaching. This applies to individual research courses as well.

  3. When the report is completed, please return it immediately to the Office of Classroom Management, 233 Buchanan Hall.
