OUNet account (exception account renewal)
This form is designed for personnel accounts only. Before each exception account expires, you and the sponsor of your account will receive three (3) emails: 30 days, 15 days, and 1 day prior to expiration. You must fill out this form to renew your account. Complete this form, print it out, and obtain appropriate approval signature. FAX the completed form to (405) 325-4339. For assistance call (405) 325-HELP or visit the Online Support Center at http://support.ou.edu. All other requests should use the Organizational Account Request form available at http://forms.ou.edu. Handwritten forms will not be accepted.
