Before each exception account expires, you and the sponsor of your account will receive three (3) emails: 30 days, 15 days, and 1 day prior to expiration. You must fill out this form to renew your account. Complete this form, print it out, and obtain appropriate approval signature. FAX the completed form to (405) 325-4339.
