Master's Degree Forms and Documents

This list includes only forms and documents specifically for master's degree students. Additional forms and documents are available here.

Approval for Thesis/Dissertation Submission to SHAREOK

Required for students defending on or after August 20, 2018. Must be submitted to the Graduate College before the student submits their final thesis to SHAREOK.

Signatures: Student, all committee members.

Deadline: All committee members must approve the final thesis before the student submits to SHAREOK. The Graduate College will not approve a SHAREOK submission until this form has been received. Although the official deadline to submit the thesis is 60 calendar days from the defense, most students will need to submit much earlier to meet the semester submission deadline, graduate in the desired semester, and avoid additional enrollment. Refer to the deadlines page for semester submission deadlines.

Authority Report Form for the Non-Thesis Exam

This form is customized individually by the Graduate College for each student and sent to the examining committee via email when the exam is authorized.

Signatures: Examining committee.

Deadline: Within one week of the committee's decision, but no later than the final day of the semester.

Authority Report Form for the Thesis Defense

This form is customized individually by the Graduate College for each student and sent to the student and thesis committee via email when the defense is authorized.

Signatures: Thesis committee.

Deadline: Within 72 hours of the defense.

Required for thesis master's students.

Signatures: Student, graduate liaison, thesis committee members.

Deadline: The semester before graduation. Refer to the deadlines page for specific dates.

Non-Thesis Instruction Packet
Required reading for non-thesis master's students. Outlines the non-thesis exam process and includes instructions for degree completion.PDF
Program of Study

Required for master's students. Lists all coursework that will be applied to the master's degree requirements.

Signatures: Student and graduate liaison.

Deadline: The semester before graduation. Refer to the deadlines page for specific dates. Graduate assistants may need to submit it earlier for tuition waiver eligibility.

Request for Authority to Defend

Required for thesis master's students. This form replaces the Request for Authority for Thesis Defense.

Signatures: Student, graduate liaison, committee chair.

Deadline: Five (5) business days before the defense. Refer to the deadlines page for deadlines to hold a thesis defense.
