• Concurrent Enrollment Letter Request

For current international students who wish to enroll on another campus for any period of time

• Departure Form for CESL Students

For CESL students who are departing the US

• Departure Form for F-1 and J-1 Students

For current international students who are departing the US either before or after their program end date

• Employment Clearance Form

For current international students who are applying for OPT, or current international students who are extending their stay and wish to extend their on-campus employment eligibility with OU payroll. Submit this form to OU ISS.

For current international students who need more time to complete their program and whose program end date is in the future

• F-1 Student Responsibility Form

All F-1 students must sign and submit this form to OU ISS

• I-20/DS-2019 Request Form

For current international students who are updating their major, changing their funding, adding dependents, or otherwise changing the information on Form I-20

• International Fee Removal Request Form

For students who have been charged the international student fee in error

• J-1 Academic Training Information Sheet

For current J-1 international students who are applying for pre-completion or post-completion Academic Training employment authorization. Complete and submit this form to OU ISS with all supporting documentation.

• J-1 Student Responsibility Form

All J-1 students must sign and submit this form to OU ISS

• Letter Request Form

Use this form to request a letter of good standing or a visa application support letter

• Reduced Courseload Request

Academic advisors must complete this form if an international student enrolls in less coursework than is required under immigration regulations. Limited reasons apply.

• Transfer Recommendation Form

Submit this form to your current institution if you plan on transferring to OU

• Transfer Release Form

Submit this form to OU ISS if you plan to transfer from OU to another US school. You must provide proof of admission to the new school before OU can release your SEVIS record.

• Travel Signature Request Form

Complete and submit this form with your I-20 or DS2019 if you plan on reentering the US from abroad on your student visa
