The Importance of Documentation:
Faculty should be aware that written information pertaining to dates, times, witnesses, the nature of the incident the impact of the disruptive behavior and action, if any, taken by the faculty member, are important in any disciplinary proceedings. Referrals to the Office of Judicial Services require written documentation containing factual and descriptive information. Once the student is charged under The University of Oklahoma Student Code for Responsibilities and Conduct, this documentation becomes part of the educational record and the student is entitled access to this entire record.
