Referral Forms

You may refer a child to OKDBTAP by completing one of the two forms below.

SoonerStart Form - is for infants and toddlers who are birth to two years old.

School-Age Form - is for students who are 3 to 21 years.

Forms must be printed and mailed to the address provided.
