HAZARD Report Form
Instructions: Fill out all appropriate sections of this form and
press the SUBMIT button to send your form as EMAIL to the OU Police Dept.

NOTE: While you can use this general form to report ANY hazard to OUPD, it will speed up service if you report certain types of problems DIRECTLY to the appropriate campus office:

For
key problems,
electrical problems, and
lighting problems (including exit, emergency lights, building-exterior and street, or general lighting) —except DARK AREAS (where new lighting is needed)
use the OU Physical Plant's
Report-A-Problem Form.

For
parking and MetroTrans problems
use the
Parking & MetroTrans Feedback Form.

For
fire equipment problems,
observed fire hazards, and,
fire alarm problems,
use OU Risk Management's
Fire Safety Unit Service Form.

If the hazard represents a life-threatening emergency, DO NOT use this form; Dial 9-1-1 and report it to the 24-hour OUPD dispatcher!!

NORMAN CAMPUS HAZARD REPORT FORM
DO use this form to report: Physical security equipment defects, security alarm system problems, hazmat problems, health-safety problems, dark areas (lighting needed), other university unit equipment defects, and general safety problems including observed occupational safety practices.
