Campus HAZARD REPORT Form
Use this form to report a building or exterior safety hazard to OUPD. (including reporting DARK AREAS where lighting SHOULD BE installed)
Physical Plant Maintenance Request Form
Use this form to submit hazards/problems regarding exterior lighting (except dark areas where lighting is NOT installed), and other trouble complaints requiring OU Physical Plant action.
(This form is on the Physical Plant web server and sends mail to the Physical Plant Maintenance office - not to OUPD)
OU Fire Safety Unit Service Form
Use this form to report fire extinguisher or other fire safety problems, or to request fire drill, fire inspection, fire extinguisher classes, or any other OU Fire Safety Unit service.
