Acrobat Reader Instructions
You must have Adobe Acrobat 4.0 reader installed on your computer in order to use any of the PDF forms. The Acrobat reader is free and installation will only take a few minutes.

PLEASE PRINT AND THIS PAGE AND READ
THE INSTRUCTIONS CAREFULLY BEFORE ATTEMPTING TO INSTALL THE READER.

Click WINDOWS VERSION OR MAC VERSION to to download your free copy of the Acrobat 4.0 Reader installer application. You may need to use the "Save to Disk" function of your Web browser. Remember where you saved the file.

After the download is complete, exit your Web browser.

Find the newly downloaded file on your system and install it by "double clicking" on the file. Follow the instructions on your screen. Restart your WEB browser once your reader has been successfully installed.

Once this is accomplished you can go to the Documents Online page and view the PDF forms by my double clicking in the icon.

Once the form appears, select the icon in the toolbar with a hand if not already selected.

The "zoom" can be adjusted by selecting either of the magnifying glass icons.

INTERACTIVE FORMS- Some of the forms are "interactive". That is, you will be able to fill-in and print out a completed form from your WEB browser.

Simply select the field you want to enter data, then type in the information. After each field hit the tab key to advance to the next field or use your mouse to advance to desired field.

NON-INTERACTIVE FORMS- To use the forms that are not interactive, simply view the form with your Acrobat Reader and print out a blank copy of the form.
