Below Practicum Liaison Faculty will find forms needed to document field visits. Please use the Initial Visit form for the first visit made and the Follow-Up form for each subsequent visit.

Faculty may use either the WORD document or the document in PDF. Click on the appropriate icon below. WORD documents may be opened, completed partially or in full, and saved. Forms in PDF are not writeable. Faculty who use them will need to type the document or complete by hand in ink. 

All forms must be signed by the liaison and submitted to the Practicum Office.
