OUSSW students are placed only at sites that are officially affiliated with the OUSSW.

To become an affiliated Practicum site for the OUSSW, staff at the site must first complete the Practicum Site Information Form. Once submitted, the form will be reviewed and, if approved, an official affiliation agreement will be developed. Below you will also find the Practicum Instructor Information Form. This form may also be found on the Practicum Instructor’s Page. All students in Practicum placement through the OUSSW must be supervised by a professionally educated social worker. All Practicum Instructors must have either an MSW or BSW or the equivalent degree in social work. A Practicum Preceptor Information Form is also available below. Practicum Preceptors are assigned on occasion in student placements. The extent of responsibility of preceptors for the teaching and learning process is negotiated individually by the assigned Practicum Instructor and faculty liaison. When a Practicum Preceptor is involved in a student’s educational program, the preceptor must complete the Practicum Preceptor Information Form and must be included in plans for the student’s learning and evaluation of performance.
