Below students will find all forms needed to complete Practicum coursework. The page is divided into two sections. The first section contains forms that are used by all students in Practicum placement. The second section contains forms that are specific to the student’s study. Be sure to use the correct Teaching-Learning Agreement, Mid-Term Evaluation, and Final Evaluation for you.
Students may use either the WORD document or the document in PDF. Click on the appropriate icon below. WORD documents may be opened, completed partially or in full, and saved to the student or instructor’s files. Forms in PDF are not writeable. Students who use them will need to type the document or complete by hand in ink. Forms completed in pencil will not be accepted.
Writing on all forms submitted must be legible and students are expected to use correct grammar and spelling. Some of the WORD forms do not permit spellcheck. Students are advised to type their statements on a blank document, spellcheck the content, and then copy and paste their work into the WORD form.
Most forms require the signature of both the student and the assigned Practicum Instructor. If the signature of the Practicum Instructor is required and the form is submitted without that required signature, the document will be considered incomplete and will not be accepted and the student may miss a deadline. NORMAN STUDENTS
Norman students are required to submit practicum forms in person or by mail. ONLY original documents and original signatures will be accepted and all deadlines will be enforced.
TULSA STUDENTS
All Tulsa students should access and submit their forms through D2L.
