Design Request Form

This form must be filled out for any design you are wanting created by professional staff.

The form should be submitted no later than 21 days before you are wanting the finished design.

Production Request Form

Use this form to submit your details for your lighting scheme for the production core group. This form will provide the production team with helpful informaiton and meeting times to go over the plan with you in person.

The form should be submitted no later than 3 weeks before your event.

Expense Request Form

This form must be filled out for any purchase (supplies, printing, catering, etc.) relating to a Union Programming Board function. Purchases may not be made without going through this process.

The form should be submitted no later than 3 business days before the purchase is to be made.

Social Media Request Form

Use this form to submit your social media plan to be posted from the official Union Programming Board media accounts.

This form should be submitted no later than 24 business hours before the first post is to go live.

Schedule a Student Office Meeting

The UPB student office is open for UPB members to schedule small meetings. To schedule a meeting, follow these steps:

  1. (If Necessary) Click 'Launch' to open the calendar.
  2. On the calendar below, click use the upper left hand corner to select the day of your meeting.
  3. Click at the time you want your meeting to start.
  4. Select the ending time for your meeting (max time of 2 hours). You may do this by dragging the bottom of the box that appears or by selecting the time above the calendar.
  5. Click the green check-mark button to confirm time.
  6. If prompted to login, use the following informaiton:
    • user: upb@ou.edu 
    • password: upb1938
  7. Add the title of your meeting (i.e. Escape CG Asst. Dir. Meeting) in the 'Title' section
  8. Add your name and email in the 'Notes' section.
  9. Select 'Confirm Booking'

Guidlines for Student Office Booking

  • Per university rules, bookings are unavailable between the hours of 11 PM - 7 AM.
  • You may not edit the booking of another individual.
  • To view who has made a specific reservation, hit the down arrow on the right of the reservation.
  • If a reservation is not properly submitted (follow the steps above) it is subject to removal.
