Student-Directed Transition Planning

Why SDTP?

How can we be sure our students with disabilities move from high school into a happy, secure, and culturally meaningful adult life? Moving through and planning for life after high school is a challenge for almost all teenagers and their families. Students with disabilities and their families have an even more difficult time understanding and positively impacting this process of transition from high school given their disability-related constraints. Yet another layer of frustration and difficulty occurs for students with disabilities and their families, who also come from culturally and linguistically diverse backgrounds.  For these teenagers, the challenges may seem insurmountable as the student moves from high school into adulthood.  Helping students with disabilities from culturally and linguistically diverse backgrounds understand and manage the transition process with their disability, while maintaining familial and cultural connections is difficult. The ability of the student and teacher to understand the impact of disability on learning and achieving future adult outcomes, within the context of the student’s unique familial and cultural background, is critical to the success of the transition planning process.
