Comments about this page:

This form takes the values from the user and places them in different variables(ex: <INPUT NAME="A. First Name"...> places your first name in the variable: A. First Name). There are 3 required input fields which MUST be in your form:

<INPUT NAME="Subject" TYPE="HIDDEN" VALUE="Email Test Form" SIZE=62><BR>
<INPUT NAME="Email From" SIZE=30>

The FORM METHOD must be POST or the form will not work correctly. The subject VALUE line may be whatever you wish. You may make them HIDDEN fields if you need to, but they must be in your form and they must be named exactly as shown above. The rest of the fields do not matter. The variable names in each INPUT line are lettered for a reason. The output email message is sorted alphabetically by those variable names. To receive a nice looking email message, letter your fields similarly otherwise you will receive an email message which is sorted alphabetically by the variable names.
