Advising and Forms

Students needing to complete an Academic Performance Contract should call 405-325-7370 to schedule an appointment with Carmen Marcom.

Each School has faculty advisors that work with students on a semester by semester basis.  Students should see their faculty advisor once a semester for advisement and enrollment.  Students can find out who their faculty advisor is by contacting the main office of each School.

Students must declare this minor in the Fine Arts Dean's office, in addition to completing this form with an Art History faculty advisor.

College of Fine Arts General Petition (pdf)
This form is used for requests not addressed by other forms already in place.

University Petition to Drop a Course After the Deadline (pdf)
This form is required to drop a class from your schedule after the official enrollment deadline. Students must also attach a "Drop Form" signed by the instructor of the class. The Drop Form is available in each School's main office or in the Fred Jones Art Center, room 122.

University Enrollment Overload Petition (pdf)
This form is for undergraduate students wanting to enroll in more hours than the allowed term limits.
