Welcome! To help us prepare for your visit, please complete our new patient paperwork and provide a copy of the front and back of your prescription insurance card.
Once completed, you can submit the forms and insurance card by fax (405-325-0153), email* (pharmacy@ou.edu), or by bringing them to our office (620 Elm Avenue) in person. We look forward to seeing you!

To help avoid delays, we encourage you to request prescription refills before your visit whenever possible. You can also sign up for text alerts to receive a notification as soon as your prescription is ready for pickup. Please note: If you would like to transfer a prescription from Goddard Pharmacy to another pharmacy, simply contact the pharmacy where you would like to fill your prescription and ask them to request the transfer from us. They can reach Goddard Pharmacy at (405) 325-5165 to complete the transfer on your behalf.
We're happy to help you make the switch to Goddard Pharmacy by accepting prescription transfers for your convenience.
To transfer your prescriptions, simply complete the form below, and we'll take care of the process for you. Most prescription transfers are completed within 1–2 business days.
Please note that we can only transfer prescriptions with refills remaining. If your prescription has no refills left, please contact your healthcare provider and ask them to send a new prescription directly to Goddard Pharmacy.

At Goddard Pharmacy, we offer a variety of convenient payment options to make your visit as easy as possible. We gladly accept cash, credit and debit cards, HSA/FSA cards, checks, and Apple Pay.
For added convenience, students may charge eligible prescriptions and over-the-counter purchases to their Bursar Account.
Please note that all pharmacy sales are final. If you have any questions about payment options or your purchase, our team is happy to assist you.
Filling your prescriptions at the Goddard Pharmacy is easy and convenient! We’re happy to fill prescriptions written by both Goddard Health Center providers and your personal healthcare provider.

Some insurance plans include an annual deductible. This is the amount you are responsible for paying toward your healthcare services each year before your insurance plan begins covering eligible expenses
An insurance prior authorization is a process where your health insurance company reviews certain prescriptions before they are covered.
Insurance companies use prior authorization to confirm that a requested medication meets their coverage requirements. If approval is required and obtained, your insurance company may then cover the service according to your plan benefits.

Goddard Pharmacy is happy to provide Pre-exposure Prophylaxis (PrEP) for HIV prevention. Have your healthcare provider send your prescription(s) directly to us, and our team will be glad to assist.
Goddard Health Center is proud to offer a safe medication disposal box located in the pharmacy for your convenience. Proper disposal of unused or expired medications helps protect our community and environment. The OU Police Department also provides a medication disposal site on campus.

What is Narcan?
Naloxone (Narcan) is a medication designed to rapidly reverse opioid overdose. It is an opioid antagonist—meaning that it binds to opioid receptors and can reverse and block the effects of other opioids, such as heroin, morphine, oxycodone and fentanyl.
A person who is experiencing an overdose can’t administer naloxone/Narcan to themselves. Anyone can carry Narcan, give it to someone experiencing an overdose, and potentially save a life. That’s why everyone should consider having naloxone.
