Treatment and Management of ADD and ADHD

How to send your ADD/ADHD records to OU Health Services

OU Health Services provides medication management for patients (1) who have been diagnosed with ADD/ADHD and (2) have documentation of an approved psychological assessment. 

If you have a current prescription for ADD/ADHD medications: You can bring your prescription to the OU Health Services Pharmacy (must be written less than 30 days from the filling date) or have the prescribing provider send the prescription electronically to the pharmacy.

For students that meet these criteria:

Please submit your records with your diagnosis, treatment, and psychological testing to OU Health Services. Once you have done this, your information will be reviewed and if approved, you will be contacted to schedule an appointment with one of our providers. 

You can easily submit your records by following these three steps:

  1. Complete this records request form (pdf)
  2. Submit this form to the provider whose records you are requesting (this is the provider who made the ADD/ADHD diagnosis and performed diagnostic testing and treatment).
  3. When we have received the records from this provider, they will be reviewed and you will be contacted to schedule an appointment.

For students who have never been diagnosed with ADD/ADHD

OU Health Services does not diagnose ADD/ADHD. If you believe you have ADD/ADHD, you will need to have approved formal diagnostic testing performed. After this testing and diagnosis, you may see a health care provider at OU Health Services for evaluation and treatment.

Testing performed off campus will be evaluated on a case-by-case basis.

Off-Campus Testing
