I-MakeUp Reports User Guide

Overview

Please log in to Apply using your OU Net ID (4x4) and password. If you have any questions or concerns, please contact Academic Records at records@ou.edu or 405-325-4147. When you log in, you will see a list of options on the left side of the screen: Dashboard, Incomplete Contracts, I-Makeup Reports, Documents, and Logout. A basic search box will appear in each tab with an advanced search option. Advanced Search allows you to search using additional fields, like course name or status.

The Dashboard

The dashboard is the landing page that displays all reports you have initiated and their statuses. Both the Incomplete Contracts and I-Makeup Reports panels allow you to view details of existing contracts or create new instances.

Contract Statuses

  • Draft: The contract or makeup has been initiated but not completed.
  • Submitted: The contract has been completed and signed by the instructor and is awaiting student signature. Note: Apply will automatically generate an email for the student to log in and sign their agreement.
  • Finalized (Step 1 of 2): The contract has been signed by the student and will remain in this status until coursework is complete and a makeup report is made.
  • Finalized (Step 2 of 2): A makeup report has been submitted; Academic Records is automatically notified by the Apply system to update the final grade.

By selecting the blue icon with the circle and i under Actions, you will be able to view the contract in its entirety and download a PDF if you wish to keep a copy for your records.

To void or rescind a contract, select the red X icon under Actions. You may also contact Academic Records to void a contract if needed.

Please ensure you have issued a final grade of "I" for the student before initiating an Incomplete Contract. If the contract agreement was made after your final grades were entered or you are not sure if you entered an "I" and would like confirmation, please contact records@ou.edu.

Incomplete Contracts

Selecting the Incomplete Contracts tab from the sidebar will display a more detailed view of all contracts. This will default in order of most recent to oldest but can be adjusted by selecting any label with arrows to organize by alphabetical order, course ID, course name, updates, or status.

I-Makeup Reports

Selecting the I-Makeup Reports tab will display the same information as the Incomplete Contracts tab without the option to void. Once a contract has been finalized, we can no longer void it; however, if you do not wish to honor the final grade or there is an issue with the submission, please contact Academic Records for adjustments.

Creating an Incomplete Contract

Select the Create Contract icon to generate a new contract.

Enter the student’s Sooner ID number. Please ensure you are using the correct ID; it will automatically populate student information based on ID.

On the Select Course page, select the blue checkmark icon to initiate a contract for the course.

Enter all information and select Sign and Submit at the bottom of the page when completed. This will automatically generate an email to the student prompting them to review and sign the agreement. You will receive an email when it has been signed by the student.

Completing an I-Makeup Report

Select the green Create Report icon to generate a makeup report.

Enter the student’s Sooner ID number and select the blue checkbox icon next to the appropriate course. This landing page will default to the current term, so you will need to use the dropdown menu on the top right to navigate to the term of the course for which you are completing the makeup form.

Once you have entered the student's Sooner ID and selected the appropriate course blue checkbox, you will land on a page that is autofilled with the current term, so you will need to use the dropdown menu on the top right to navigate to the term of the course for which you are completing the makeup form.

On the Finalize I-Makeup Report screen, enter the final grade and date the coursework was completed.

If the student has not turned in the remaining coursework before the contract has expired, the final grade should reflect the grade the student earned with what they did complete and turn in and the date the coursework was completed should reflect your last communication with the student or the last day of the course.

Once a makeup form has been submitted, a notification is automatically sent to Academic Records. Once Academic Records has updated the grade, the report is filed and no further action is needed.

Incomplete Contracts & I-MakeUp Reports FAQ

Please review the Grading Policies for guidelines and restrictions on issuing an incomplete.

An incomplete cannot replace a failing grade, and no student may be failing at the time the contract is issued. The student must have completed 70% of the coursework and be actively passing the course at the time of agreement.

The contract can be any duration of time appropriate to complete the coursework but may not exceed one calendar year.  

All contracts and makeup reports are filed after completion and archived within Apply. If you need information on a prior contract, please contact Academic Records for assistance.
